AHG HEALTH FORM

YOUTH and ADULTS must have this form completed and on file at camp. This form should be on file with your local troop. A copy is sufficient. MUST BE SENT IN BY JUNE 21st.

MEDICATION ADMINISTRATION FORM

Anyone who needs any medication at while at camp needs to fill this form out and bring to nurse during registration. Please have all medications in original bottles and placed in gallon size ziplock bag with this form inside.

HIGH ADVENTURE FORM

The High Adventure form must be filled out for a number of Badges. We will let you know if you need this form once your badge schedule is complete. **Note – This form requires a doctor’s signature, so don’t put it off until last minute! MUST BE SENT IN BY JUNE 21st.

SWIM TEST FORM

All swimmers or girls that participate in Badges that are in the water, must have this form on file at camp. You can have the form completed by a certified lifeguard, water safety instructor, or swim coach prior to camp or on Sunday evening at camp. All girls wanting to swim during the week, must take a swim test on Sunday evening due to having to have a Clyde York swim test, as well as an AHG swim test.
